To register for your patient portal account for Trinity Medical Group, please provide your photo identification and your e-mail address to the registration professionals at your Trinity Medical Group provider’s office (offices located at Coshocton, Dennison, Dover, Gnadenhutten, Newcomerstown, & Uhrichsville).
