Register now to join us for this free support group meeting for those with diabetes and their family/friends who support them.

The March meeting topic is "Trinity Medical Group's Chronic Care Program." Learn how this program can help you manage your diabetes and also help you coordinate your care with other healthcare providers and community agencies to ensure you are reaching the best quality of life while living with diabetes. This meeting will also provide information from a panel of people who participate in the chronic care program. They will discuss how the program has benefited them.

This group meets on the third Monday of every month for about one hour at the St. Raphael Conference Room at Trinity Hospital Twin City's second floor. The meeting is led by Certified Diabetes Educator, Lu Ann Beavers-Willis, MSN, RN. For more information, call (740) 922-7450, ext. 2177.
